At Recovery Hub, we help individuals navigate treatment, access community resources, and stay connected to the services that promote long-term health, stability, and recovery.

What Is Care Coordination?

Care Coordination, sometimes called Treatment Coordination, is a collaborative service that helps individuals access, organize, and manage the healthcare, behavioral health, recovery, and community resources needed to support their well-being. Recovery journeys can involve multiple providers, treatment programs, support services, and personal goals. Care coordination helps ensure these services work together in a way that is effective, organized, and centered around the individual’s needs.

Our team works alongside clients to identify barriers, coordinate services, and create a plan that supports ongoing recovery and overall wellness.

Our Approach to Care Coordination

At Recovery Hub, we believe recovery is most successful when individuals have access to the right support at the right time. Our Care Coordinators take a person-centered approach, working closely with each individual to understand their goals, strengths, challenges, and priorities.

We focus on building strong partnerships, advocating for client needs, and helping individuals navigate the systems and resources that support lasting recovery. Rather than simply making referrals, we help create meaningful connections that empower individuals to move forward with confidence and stability.

  • Counseling focuses on therapeutic support and personal growth, while Care Coordination focuses on connecting individuals to services, resources, providers, and supports that help them achieve their recovery and wellness goals.

  • With your consent, Care Coordinators may work with healthcare providers, treatment programs, community agencies, and other support services to help ensure coordinated care.

  • Care coordination is available during your entire stay in our outpatient programming and is based on individual needs, goals, and circumstances. Support is tailored to help individuals achieve greater stability, independence, and connection to ongoing resources.
